Device Loan Agreement Terms

By signing below, I acknowledge and agree to the following terms regarding the device(s) loaned to me by CARDBoard Project through the Digital Access Program (DAP):

I understand that the device(s) are on loan and remain the property of CARDBoard Project.
I am responsible for the proper care, use, and safe keeping of all loaned device(s).
I agree to return the device(s) upon request or at the end of the loan period in the same condition as received, normal wear and tear excepted.
I understand that I may be held financially responsible for any damage, loss, or theft of the device(s) during the loan period.
